Here is an opinion question..

If your hand size is exactly on the boundary between Small and Medium (for example) is it better to have the smaller grip and palm shelf all the way out of the larger size with it all the way up?

My opinion on the matter;
If you like or want to do some fitting of the grip to suit you, go with the small and work it to fit your hand correctly. It's much easier to remove material than add it back on and have it look nice.
If you don't want to sand and grind a bit, buy the larger and crank the palm shelf up to fit. Add some putty and you'll be in business.

That said, I prefer a high volume palm swell with a deep cut for my callused hand (I go with the larger size every time).

The difference is very small so neither palm shelf should be at the extreme. I'd personally go larger but either would work. Don't forget almost everyone's hands are different so chances are you will need to adjust any grip you get.

The smaller size will--in addition to the adjustable palm rest difference--often have a smaller girth as well; that should also be taken into account for best fit.
